Home Health Aide / Personal Care Aide

Position Description:

Perform duties essentially of personal care, which might be given to waiver participants in their homes. While working with individuals and their families assist the participants in assuming responsibilities toward self-care and independence. Help the well family members to carry their share of the responsibilities and encourages the disabled member to participate in the life of the family, in so far as able.

Hours: Morning, Evening, and Overnight Shifts available.  Full and part time employment available

Responsibilities:

1. Maintain the standards and policies set forth by Access to Home Care Services, Inc.
2. Work under the framework of the consumer and the Care Plan.
3. Assists the participant with some of the following, not to include or exclude all as assigned in the Care Plan: Transferring and positioning, toileting, dressing, meal preparation, feeding, laundry, housekeeping/cleaning, bathing, shaving/make-up/hair styling, utilization/positioning of adaptive equipment, transportation, recreation, grocery shopping, shopping for personal items/clothing, exercises/stretching, and/or other duties as assigned by the consumer without performing any task that is outside of the PCA scope of practice.
4. Maintains confidentiality and does not discuss issues about the participant with anyone else. Respects the participant's home, possessions, lifestyle and individual choices.
5. Participate in Team Meeting.
6. Report any needed changes to Service Coordinator.
7. Complete necessary documents in accordance with Agency and Waiver policies.
8. Respect and enforce Client/ Patient Bill of Rights

Received: Case Manager, Service Coordinator, Participant/Representative

Minimum Qualifications:

1. Must be High School Graduate or have GED and /or Possess HHA/PCA Certificate.
2. Complete and pass a Fingerprint and background check.
3. Excellent verbal and written skills
4. Be emotionally and mentally mature
5. Ability to take direction
6. Ability to interact with clients
7. Ability to; speak, read, and write the English language.
8. Maintain current health exam and immunizations.
